We're Settling In, Opening our Procedure Center and Streamlining Visits!
Hi everyone! We’re so excited to be settling into our new office! Thank you all for your patience during our recent move. We’re thrilled to announce that we are launching our NEW Procedure Center!
In addition to that, we’re implementing some new office policies to streamline your visits. Please be on the lookout for notification emails prior to your scheduled appointments (excluding pellet appointments). These emails will alert you to log into our patient portal to complete your MRS and RAMS forms, along with a NEW pre-appointment questionnaire. We appreciate your cooperation as we implement these improvements. For more details, see below!

Prior to your scheduled appointments, you will be asked to complete a brief set of forms in the patient portal. Completing these forms beforehand will help us streamline your appointments and ensure we address your specific needs efficiently.
These forms will include:

RAMS Form: For male patients
Pre-appointment Questionnaire: For all patients

Ensure ideal treatment plans: Your information ahead of time will help us tailor your care plan.
During your appointment, we will prioritize:
Review of test results: We’ll discuss any recent blood work, urine tests, or specialty lab results.
Treatment plan adjustments: We’ll review and adjust your current treatment plan, including medications, hormone therapy, and supplements.
